In 2320, Jacob Lee (Josh Duhamel) and Max Barrow (Jeff Schenn) work as contract freighters for the United Jupiter Corporation (UJC). After their final transport from Europa to the Black Iron prison on Callisto, their ship Charon is hijacked by the Outer Path, a terrorist group believed to be behind a devastating biological attack on Europa. The spacecraft crashes on Callisto, killing Max, leaving Jacob and Alien Path leader Danny Nakamura (Karen Fukuhara) as the only survivors.
They are captured by Captain Leon Ferris (Sam Witwer) and imprisoned on the orders of Warden Duncan Cole (James C. Mathis III). However, the prison is overrun with infected prisoners known as “biophages,” and Jacob forms an alliance with prisoner Elias Porter (Zeck Alton) to find an escape route. They encounter Special Housing Unit (SHU) hacker Dani, but she initially refuses to join their escape plan.
As Jacob and Elias made their way to the hangar, they were attacked by an infected Ferris and thrown onto Callisto’s surface. Elias is mortally wounded and Danny saves them with his car. She restores Elias’ memory, and together they work to survive as the biophage becomes more dangerous. Mahler reveals that Cole is a member of a religious organization called Commonality, which is dedicated to promoting human evolution. She reveals Cole’s plan to use Jacob and the alien infection to create an Alpha subject, and tells Jacob that Danny can be cured by extracting Alpha’s DNA.
As Jacob investigates further, he discovers that he was involved in the UJC operation because he knew about larval smuggling but ignored it for personal gain. He confronts Cole, who releases a mutated Ferris (now Alpha) to stop Jacob. However, Jacob defeated Ferris and extracted his DNA.
Cole urges Jacob to hand over the DNA for the sake of humanity’s survival, but Jacob instead uses it to heal Danny. Enraged, Cole initiated the prison’s self-destruct sequence, claiming to have collected enough data for the Callisto Protocol. Jacob puts Dani in the escape pod along with the alien larvae sample, providing her with evidence to expose the UJC experiment. With the self-destruction temporarily postponed, Mahler contacts Jacob for a chance to escape, but before he can reach safety, he is attacked by Ferris.
