Her father, Eric Shapiro, a businessman, and her mother, Michelle Shapiro, a businesswoman, raised her and her sisters, Abigail Shapiro and Abi Monterey, in Tampa. She was born with cleidocranial dysplasia, a condition that causes her to have a disability.

Milly Shapiro began practicing her singing skills when she was three. As she grew older, she developed a love for acting.
She began performing at a theater school two years later and appeared in most of the productions there. After being discovered by an acting agent while she was attending the Community Montessori School, Milly Shapiro and her family moved to New York to act in her first Broadway production.
In the summer of 2020, Milly Shapiro graduated from high school and it looked like she would be focusing on her acting career instead of going to college.

Milly Shapiro rose to fame after playing Matilda Wormwood in the musical Matilda the Musical at the Sam S. Shubert Theatre from March 8, 2013 to January 19, 2014.
The play, based on the book of the same name by Roald Dahl, tells the story of a five-year-old girl who can control objects with her mind but struggles with family and school issues. Milly Shapiro received a Tony Honor of Excellence for her performance and was nominated for a Grammy Award for Best Musical Theatre Album.
Live Out Loud – Live at 54 Below, a live album that Milly Shapiro and Abigail Shapiro recorded while they were in 54 Below, was released in 2014. In the musical comedy, You’re a Good Man, Charlie Brown, which opened at the York Theatre in 2016, Milly Shapiro plays Sally Brown, a character created by Charles M. Schulz for his Peanuts comic strip.
In the horror-mystery-drama Hereditary, starring Toni Collette and Gabriel Byrne, about a family that has experienced loss and is now haunted by strange and inexplicable events, Milly Shapiro made her acting debut in 2018. She played Charlie Graham in the film.
